MHFA Car Rental Request

If you are flying to training, request air travel first here. The car rental pickup and drop off will be coordinated with your co-trainer and flight times, and a quote will be provided to MHFA for approval.

Reason for a car rental vs. Uber/Lyft/ Taxi? (less expensive/lack of service/etc) If you need a larger vehicle, include requirements here and provide details (terrain, advised by host, etc.) If car rental approval was included in your Shiftboard assignment, you may note that here but you will still need the quote generated from this form for final approval.
